The report claims that advanced negotiations have taken place in the last 24 hours and Chelsea have agreed to pay a €45 million (£37.8m) for the Morocco international.
Hakim Ziyech has already played against Chelsea twice this season and has been a key figure at Ajax over the past two seasons with 29 goals and 45 assists in 78 appearances.
